Summer membership forms are required for everyone. And must be filled out annually. Along with taxable income verification.

This form is for any additional Health, Mental, Physical Issues, Medications, reactions, other that can help your member(s) succeed in our program and help us evaluate their level of need. Memberships are not guaranteed.
All forms must be submitted and processed before you can register for weekly camps.

Is your child ALREADY a member for Summer 2023? Just fill out the commitment form, and you're all set!
Is your child NEW to our programs? Is your child RETURNING for the 23-24 School Year, but NOT enrolled in Summer Programming?? Just fill out the membership form and you're all set!
Whether you're NEW or RETURNING, all families with #FFYKids in FIRST GRADE & UP must fill out the Indiana Kids Form. This program helps with reading and math skills, as well as helps the Club at FFY with resources for high quality programming!

BOYS & GIRLS CLUB
at Foundation For Youth
At Foundation For Youth, we are dedicated to creating a positive place for our FFY Kids through different aspects of programming.
We create meaningful programs during the After School Season which include time in our state-of-the-art Maker's Studio,
Comcast xFinity Computer Lab, Cranium Bowl, Game Room, Playground, Gymnasiums and MORE!
During the Summer Season, it's FFY Summer of Exploration where FFY Kids discover a world of possibilities and friendships with special field trips, time away at Columbus Youth Camp, and so much MORE all culminating with an FFY Kid Talent Show and End of Summer Carnival.
During fall/spring break time, FFY is here to provide even more programming so our FFY Kids will get the most out of their time off. Games, scavenger hunts, staff versus club member basketball and so much MORE are awaiting our FFY Kids!
And all that is just for our FFY Kids in Kindergarten to Sixth grades! Check out our #FFYTeens section for the most active teen programming in our community.
At the Boys and Girls Club at Foundation for Youth, we serve all school aged youth from 5-18 years old.
