To get started as a referring Youth Service Agency or Provider, please complete and submit this MOU along with any referral paperwork. The MOU only needs to be completed once.
As a youth-serving agency you can refer families to FFH camps. This should be completed alongside potential Caregivers. FFH will keep your referral on file until we recive a Caregiver Application and find the right date/support.
The FFH Caregiver application is required before a family arrives at camp. Confidentiality is very important. This information will be shared with FFH Staff and care providers ONLY in order to best pair and serve your family.
